How do I get my medical records from the USC? ›

If you are interested in obtaining a copy of your medical records, please contact the student health center health information management on (213) 740-0206 or fax (213) 740-4961.

Criteria for Admission

For all applicants, an undergraduate grade point average of 3.0 (A=4.0) is expected and three letters of recommendation are required. For questions or to make an appointment with a faculty advisor, call (323) 442-3004 or send an email to Dr. Julie Nyquist, academicmedicine@med.usc.edu.

For USC users with a @med.usc.edu email address: click the Sign in with your @med.usc.edu Email button. The ETC is open 24 hours a day and staffed by either an emergency medicine or internal medicine physician. How to be seen at the ETC: For urgent medical issues, call: (800) USC-CARE or (323) 442-5100.
